Air freight loss or damage claims

Please fax or mail the Fill-in Claim form to Cargo to make a claim for a lost or damaged airport-to-airport shipment carried by US Airways.

The Fill-in form allows you to enter information while the form is displayed by Adobe Acrobat and then print the completed form for your records and for filing with US Airways. The Acrobat Reader does not allow you to save your fill-in form.

In addition to this form, the following support documentation will need to be submitted:

• Copy of air waybill
• Invoice for lost or damaged goods
• Invoice for repair of damaged goods, if applicable
• Carrier's inspection report
• Statement of salvage value, if applicable

• Fill-in Claim form
